Surgery Consent Form at Southwest Veterinary Center

Dear Client,

Your pet is scheduled for an upcoming surgery. To help make the day as easy and stress-free as possible, we have put together a few simple forms for you to review and sign in advance. The following forms are available on our website:

• Pre-Anesthetic Blood Testing Information Form
• Surgical Information Sheet

Please review all forms carefully. If you have any questions, you may call or email us at (832) 535-2387 or southwestvetcenter@gmail.com. On your pet’s surgery day, you will also be asked to review and sign an Authorization/Estimate Form. We will confirm that we have the correct phone number to reach you during the procedure.

The night before surgery, please do not give your pet any food or treats after 10:00 p.m. Water is fine to leave out. If your pet takes any medications, vitamins, or injections, withhold the morning dose unless the doctor has instructed otherwise.

At the time of your pet’s scheduled surgery, the doctor will answer any remaining questions and collect the completed Authorization Form.

If you elected to have any recommended blood tests performed, they will be collected and completed before the start of surgery if not done ahead of time. If any concerns arise, the doctor may contact you at the number listed on the Authorization Form.

After the surgery, the doctor will process your payment, review all discharge instructions with you, and provide a written copy to take home. If anything is unclear, please ask for clarification at any time.

We know surgery days can be stressful. Southwest Veterinary Center appreciates the trust you place in us, and we aim to make the experience as smooth as possible for both you and your pet.
